    • Despite “super clean” MRI, Pagán headed to IL
Reds placed reliever Emilio Pagán (triceps soreness) on the 15-day IL.

Advice: Despite Pagán’s MRI on Tuesday coming back “super, super clean,” the Reds decided to move the reliever to the IL while he deals with some lingering soreness. It’s unclear how long the 33-year-old will be out, but he’s been solid for the Reds this season with a 4.19 ERA and 1.34 WHIP in 19 1/3 innings while striking out 25 and walking seven batters. • Lit up in Vegas rehab start
Medina (knee) started Triple-A Las Vegas' loss to Tacoma on Wednesday, allowing five earned runs on four hits, a walk, a wild pitch and a hit batsman over 2.1 innings. He struck out one.

Advice: Medina moved up from the Athletics' rookie-level AZL club after opening his rehab assignment there May 15, and as his line indicates, the results were far from encouraging. Medina had a similarly difficult time in his first rehab start against a much lower caliber of competition, so it's clear he has plenty of kinks to work out before a move to the big-league club can be considered. Given Wednesday's results, it remains to be seen if Medina will be shifted to a lower-level affiliate for his next rehab appearance in an effort to allow him to build some confidence and momentum. • Cody Bradford (back) moved to 60-day IL
Rangers transferred LHP Cody Bradford (back) to the 60-day IL.

Advice: The team needed space on the 40-man roster to call up Jesus Tinoco with Jon Gray (groin) going on the IL. Considering the Rangers are also expecting to get back Max Scherzer, Tyler Mahle, and Jacob deGrom at some point over the summer, Bradford may not have a spot in the rotation when he is eventually healthy enough to pitch. He can be cut in all redraft fantasy leagues. Rotoworld Today, 8:56 am

    • Braves option Daysbel Hernández to Triple-A
Braves optioned reliever Daysbel Hernández to Triple-A Gwinnett

Advice: Hernández will make room on the roster for AJ Smith-Shawver, who is starting on Thursday. The right-hander will return to a 1.65 ERA and 22/9 K/BB over 12 appearances and 16 1/3 innings at Triple-A. He figures to get another shot with the big-league club soon. Rotoworld Today, 8:40 am
    • Shifted to 60-day IL
The Rangers transferred Bradford (ribs) from the 15-day injured list to the 60-day IL on Thursday, Evan Grant of The Dallas Morning News reports.

Advice: The transaction clears a spot on the 40-man roster for right-hander Jesus Tinoco, whose contract was selected from Triple-A Round Rock. Bradford recently experienced a setback with his ribs injury and now won't be eligible to return for several weeks. Rotowire.com Today, 8:31 am

    • Goes on IL with triceps injury
The Reds placed Pagan on the 15-day injured list prior to Thursday's game against the Padres due to right triceps tightness.

Advice: Pagan hadn't pitched since Sunday due to the injury, and though an MRI on his arm recently came back negative, the right-hander apparently didn't respond well to playing catch Tuesday. He'll be shut down for the next two weeks while recovering from the injury, depriving the Reds of one of their middle-relief options in front of closer Alexis Diaz. • Sent back to Triple-A
The Mariners optioned Bolton to Triple-A Tacoma on Thursday.

Advice: Bolton will cede his spot on the 26-man roster and in the bullpen to right-hander Mike Baumann, whom the Mariners acquired from the Orioles on Wednesday. In 15 relief appearances for Seattle this season, Bolton has compiled a 4.41 ERA and 1.29 WHIP across 16.1 innings in relief. Rotowire.com Today, 8:21 am
    • Back in majors
The Giants recalled Avila from Double-A Richmond on Thursday, Alex Pavlovic of NBC Sports Bay Area reports.

Advice: Avila was optioned Wednesday but will return to the 26-man active roster one day later to replace lefty Blake Snell (personal), who was placed on the paternity list. Manager Bob Melvin will have another relief option at his disposal while Avila is with the big club, but the 26-year-old will likely be sent to Triple-A Sacramento once Snell rejoins the team. • Falls to Friars
Martinez (1-3) took the loss Wednesday as the Reds were downed 7-3 by the Padres, coughing up five runs on eight hits and a walk over 4.2 innings. He struck out two.

Advice: Making his fifth start of the season, Martinez served up a leadoff homer to Luis Arraez that proved to be a bad omen for the rest of the game as he lasted only 66 pitches (40 strikes) before getting the hook. The right-hander has been volatile in a swingman role for Cincy, posting a 4.81 ERA, 1.28 WHIP and 31:6 K:BB through 43 innings, but with Nick Lodolo (groin) set to rejoin the rotation Monday, Martinez should be headed back to the bullpen.
